The county-published English birth application follows the Bureau route

The English birth application linked by Highlands is a state Bureau of Vital Statistics form. Its mailing destination is the Vital Records Section, P.O. Box 210, Jacksonville, FL 32231-0042, and it has its own Bureau instructions and fees. This is a different route from a local Sebring certificate request. Confirm which office you intend to use and obtain its current form and payment instructions before posting the application; do not combine the Bureau address with a county fee schedule.

The Spanish birth form identifies the local Sebring office

The Spanish application linked by the county identifies DOH-Highlands at 7205 South George Boulevard in Sebring. It is an older local form, while the English link leads to a Bureau form. Confirm the current local application and charges with Vital Statistics rather than treating the two downloads as identical instructions in different languages. Completing a Spanish application also does not automatically provide a translated certificate for an overseas recipient.

Current birth-record privacy and format rules still apply

Florida birth records less than 125 years old are confidential. A registrant may request their record at age 18; other requesters must meet the applicable parent, guardian, representative, court-order or authorized-release requirements. Confirm supporting documents before another person requests the certificate. For older births or a particular format, check availability with the custodian. A photocopy certificate officially produced by Vital Statistics is different from a home photocopy taken to a notary.

Florida permits adults to request a death certificate without cause-of-death information. The ordinary Official Records search is not a certified copy

Highlands describes its online Official Records database as dating to 1981 and provides document details such as parties, dates and book/page references.
