Utah document preparation depends on the issuing office, the signature and the destination. Miami Dade Apostille helps review and coordinate eligible birth, marriage, death, court, school, notarized and corporate documents for international use. Before you start
- Identify the destination country and confirm whether the Apostille Convention applies between that country and the United States.
- Obtain the correct certified record from the responsible issuing office — some offices require a direct applicant request.

Who issues a Utah apostille?
- The Office of the Lieutenant Governor issues Utah apostilles and authentications.
- Its contracted ApostilleXpress platform manages requests and delivery; the platform does not replace the state issuing authority.
- An apostille verifies official origin, not the truth of a document's contents.
- Confirm the destination and receiving office's requirements before choosing the route.
Obtain vital records directly before apostille coordination
- Utah Vital Records expressly prohibits third-party vendors from ordering certificates on customers' behalf.
- An eligible applicant must obtain the record directly.
- We can explain preparation requirements and coordinate apostille processing after you have the appropriate certified record; we do not bypass the issuing office's eligibility rules.
Choose the correct certificate or court record
- Vital Records provides birth and death certificates, with eligibility and identification requirements.
- Its marriage and divorce record coverage is 1978–2010; contact the county clerk or district court for other years.
- A divorce record from Vital Records should not be assumed to replace the certified court decree requested by an overseas authority.
Public records and private signatures take different paths
- Certified Utah public records generally do not require notarization.
- Obtain court records and divorce decrees from the District Court Clerk, marriage records from Vital Records or the County Clerk, and an official signed single-status document from the County Clerk.
- Private powers of attorney and affidavits need proper Utah notarization.
- Local police or sheriff clearances also appear in the state's notarized category.
- Corporate copies must be certified by Commerce and notarized at that department; emailed printouts are not accepted.
School records: check the signer and school category
- Utah lists public and charter school documents as eligible without notarization when they show the signing official's signature, name and capacity.
- Private school documents and other academic records are listed in its notarized category.
- Ask the registrar and authentication office which preparation applies to your particular diploma or transcript before ordering.
University of Utah students must order their own transcripts
- The University of Utah does not accept third-party transcript requests.
- Students can order their own transcript and designate an organization as recipient.
- For an accompanying form that requires a registrar or school official signature, the university instructs students to choose paper mailed delivery and upload the form.
- These are transcript-ordering instructions, not confirmation that every transcript is apostille-ready.
- Check any required certification or notarization separately before ordering; do not apply this workflow to diploma requests.
Mail-in requests now start online
- For mailed documents, first submit the state's online request.
- Then send the originals with the printed online order delivery form and follow the order's payment and return instructions.
- In-person requests use the paper form and require an appointment.
- International adoption requests have a separate in-person form.
- Use the current official request page rather than an old downloaded form.
Digital delivery has important limits
- Digital apostilles are not available for birth or death records or any documents issued by the Utah Division of Vital Records and Statistics.
- Eligible documents may have digital or paper options.
- Before ordering, clarify the receiving authority's submission procedure and whether it needs the underlying paper record.
Delivery addresses and return planning

- USPS: Office of the Lieutenant Governor, PO Box 142325, Salt Lake City, UT 84114.
- The physical drop box is discontinued. Mailed documents must be mailed back, rather than collected in person.
- Include tracked return arrangements and retain the tracking details.
- Check current office instructions before dispatch.
Compare the complete cost and timeline
- Check the current state fee schedule for your submission method.
- Digital delivery carries a separate contracted-provider charge in addition to the state fee; do not compare the state electronic fee alone with the total price.
- Record issuance, preparation, government processing and shipping are separate stages.
- Appointment availability and processing estimates are not guarantees of an overall completion date.
Authentication is not a blanket acceptance guarantee
- Utah uses a single certificate that serves as an apostille when the Convention applies and as an authentication otherwise.
- Additional federal or consular steps may be required for a non-Convention destination.
- The recipient may also require a particular record format, recent issuance or translation.
- Confirm those requirements before submission.
Notarization, translation and federal documents
- Prepare a complete notarial certificate and required original signatures before submission; the Lieutenant Governor's office does not provide notarization or translation.
- Utah does not require English translation merely to process a document, but the overseas recipient may require a translation.
- Out-of-state records follow their own state's process.
- FBI and other federal agency records require the appropriate federal route, not Utah authentication.
