  • Your ORI number — see the AHCA ORI numbers below, or check your application or facility instructions if a different agency requires the screening.
  • Payment — $40 rolling/capture fee per card. This is the capture fee only; confirm any additional program or state fees with AHCA before your appointment.

Common AHCA ORI Numbers

Your ORI number tells the fingerprinting system which agency should receive your results. Most AHCA-regulated providers use the general AHCA ORI, but a few categories use their own:

Not sure which one applies to you? Your online application usually has the correct ORI number pre-filled, and it is listed in the instructions on a paper application. If your license or facility type is not licensed by AHCA — for example, it falls under the Florida Department of Health or another agency — you will be given a different ORI number to bring. Just bring whatever ORI number your application or facility provided, and we will use that one.

Clearinghouse Screening Requirement

AHCA screenings conducted through the Care Provider Background Screening Clearinghouse require electronic fingerprints captured by an approved Live Scan provider, along with a photo. A paper FD-258 card alone does not complete an AHCA Clearinghouse screening. If you are out of state or need to use a paper card, confirm the correct authorized submission route with AHCA before your appointment.

Understanding the Fees

Our Fee vs. AHCA's State Fee

Our $40 charge covers rolling or electronically capturing your fingerprints — it applies the same way whether your prints are taken on a paper card or through Live Scan. This is the capture fee only and does not include any applicable state, federal, or program processing fees.

AHCA's own Clearinghouse screening fee is separate and varies by program. Confirm your exact itemized total — including any state or federal fees — through your CHAI account or with AHCA directly before your appointment. Fingerprint Retention & Renewal

Under Florida law, fingerprints on file with AHCA must be retained every five years. The retention window and process depend on your program — follow your employer's guidance and the current Clearinghouse portal instructions for your specific expiration date. Missing the retention window means starting over with a full new screening, so renewing on time is important.

The 75-to-15-day retention window referenced in DOH guidance applies to Department of Health retention requirements and may not apply universally to all AHCA employment programs. Follow your employer's instructions and the current portal for your specific program.

The Fingerprinting Process

  1. Get Your ORI Number

    Obtain your ORI number from AHCA, your employer, or your facility before your appointment. Initiate the appropriate Clearinghouse request through your program before visiting, and keep your TCN (Transaction Control Number) for your records.

  2. Schedule Your Appointment

    Book an appointment at either Miami Dade Apostille location, or walk in to the Biscayne office.

  3. Bring Your ID and ORI Number

    Arrive with a valid photo ID and your ORI number on hand. Bring two forms of ID if your program instructions require it.

  4. Fingerprints Are Captured

    Fingerprints are captured electronically through Live Scan, or rolled on a paper FD-258 card. AHCA Clearinghouse screenings require electronic capture and a photo from an approved Live Scan provider — confirm your program's requirements before your appointment.

  5. Results Are Sent for Review

    Electronic results are transmitted to AHCA through FDLE for review. Processing time is determined by AHCA and FDLE once your fingerprints are submitted.
