- Its dedicated location page lists P.O. Box 1000, Quincy, FL 32353 for mail, matching the birth page's Vital Records Section instructions.
- Confirm the current application, payment and mailing details before sending.
- General building hours extend to 6:30 p.m. on Wednesday, but the location page expressly limits services after 5 p.m. that day to WIC.
- Those extended hours do not create an evening certificate service.

A birth-record error needs correction before the final package
- The local birth page directs applicants with errors to the State Office of Vital Statistics in Jacksonville at 904-359-6900, extension 9005.
- Amendment forms are available from the state or DOH-Gadsden; the local instructions say the amendment form must be notarized.
- Confirm the specific correction procedure before ordering the final certificate for authentication.
- An apostille authenticates the official signature; it does not amend an incorrect name, date or other fact on the record.
Death certificates: choose the right version and correct the record first
